Minoxidil-Related Renal Function Deterioration and Pulmonary Edema: A Case Report and Literature Review

چکیده
Minoxidil is a highly effective antihypertensive agent for patients with refractory hypertension. Long-term use of a high dose of minoxidil may cause several side effects. However, the concurrence of acute pulmonary edema and rapid progression of renal function after short-term and low-dose therapy is not common. Herein, we report the case of a 70-year-old woman who had diabetes mellitus and mild impairment of renal function. Low-dose minoxidil (2.5 mg) was prescribed to manage her poorly controlled blood pressure. The dosage was gradually increased to 5 mg/day. In the nineth week after the initiation of minoxidil therapy, the patient experienced weight gain (5 kg), orthopnea, and exertional dyspnea. Physical examination revealed basal crackles in both lung fields. Chest X-ray showed bilateral pleural effusions and pulmonary congestion. One week after the discontinuation of minoxidil, her body weight returned to the previous status and the symptoms of pulmonary edema subsided completely. The pleural effusion resolved gradually two weeks later. However, her renal function deteriorated rapidly and did not return to normal. This case alerts us that minoxidil should be used with caution in patients with impaired renal function. Fluid status and renal function should be closely monitored. Whenever complications develop, minoxidil should be discontinued. (Acta Nephrologica 2011; 25: 33-36)

PULMONARY BLASTOMA: REPORT OF A CASE AND REVIEW OF THE LITERATURE
We describe a case of pulmonary blastoma, a rare primary lung malignancy which affects a younger population. This tumor contains both mesenchymal and epithelial elements. Pulmonary blastoma tends to relapse locally and metastasizes mainly in the first few years. It has a poor 5-year prognosis, and shows a potentid radio-chemosensitivity. Because these tumors are considered malignant, surgic...
متن کاملSarcomatoid Carcinoma of Renal Pelvis Involving Ureter and Renal Parenchyma with Heterologous Osteosarcomatous Differentiation: A Case Report and Review of Literature
Sarcomatoid carcinoma is a high‑grade rare malignant tumor with both epithelial and mesenchymal components. Sarcomatoid carcinoma in the upper urinary tract is very rare. We reported here a case of sarcomatoid carcinoma of renal pelvis with osteosarcomatous differentiation, with involvement of the ureter and renal parenchyma in a 68-year-old female. Histologically, predominant pleomorphic spind...
متن کاملRenal Capillary Hemangioma Mimicking Urothelial Carcinoma, A Case Report and Review of the Literature
Renal hemangioma is a rare tumor which can be capillary or cavernous. There have been less than 30 renal capillary hemangioma cases reported in the English literature. Herein we will report a case of renal hemangioma which was detected in a 74-year-old man operated with the impression of urothelial carcinoma of hilum.
متن کاملProlonged Hemoptysis Caused by Primary Pulmonary Epithelioid Hemangioendothelioma; A Case Report and Review of the Literature
Epithelioid hemangioendothelioma is a vascular tumor with an intermediate malignant potential. This tumor is very rare in the lung parenchyma, and most of the previously reported cases have been asymptomatic. There is no standard therapy for this tumor and prognosis in the previous reports has been variable.Herein we report our experience with a 60-year-old woman presenting with hemoptysis and ...
متن کاملReversible renal failure due to captopril in a patient with transplant artery stenosis. Case report.
A patient who was subsequently shown to have stenosis of the artery to a transplanted kidney sustained three episodes of impaired renal function during treatment with low doses of captopril. On each occasion this was reversed by withdrawal of therapy. The effect was not related to reduction of blood pressure since lower pressures were recorded during treatment with minoxidil, without deteriorat...
